Service Description

Follow up visits provide on going support to ensure that home modifications and safety recommendations are effectively implemented, and to support you to achieve goals set up in an assessment. We will review progress, address any new concerns, complete activities and training as needed. I will provide continued guidance on progression, education, and support needed to achieve as much safety and independence as we can!
