Service Description

As a licensed occupational therapist, I provide comprehensive home and functional evaluations to support safer, independent living. This service includes in depth assessment of your abilities, daily activities, and home environment. After assessment we will come up with a plan for care, providing guidance and actions as well as planning for future follow-ups as needed to implement plan and work toward your goals.
